Bedding in Australia

Personal preference.

I never use a sheet set, have always used a quilt/duvet and fitted sheet on the mattress.

They sell plenty of options here, sheet set, fitted sheet, quilt covers, so not sure what your dilemma is. Just get what suits your needs.

I'm confused, should I be buying a 'quilt cover' a.k.a duvet cover as is in Europe or should I be buying a 'sheet set' and basically have a flat sheet under the duvet as is the bedding in North America? ..

It's up to personal preference but the terminology and sizing is slightly different here.  A duvet is normally called a dyna (or doona) or quilt in Australia.  The American style cover is usually called a comforter or coverlet here.

A guide to bedding sizes here:

Note that Queen and King sizing are different in Australia to the UK.
