If you have a medical condition or issue that could be a reason to have a visa declined, you really should seek professional advice from a decent migration agent. As said above, that agent would be a good first point or any of the regulars that post on here could probably give your case the once over. 

If you apply for the visa and its declined, you would not get the fee back. Its money gone.

You'd need to contact the migration agents to see what their fees are.  Medical issues are considered based on the anticipated cost over 5 years (or potentially lifetime).  If it's something relatively low cost (e.g. high blood pressure), then simply declare it.  If it's something more significant (e.g. cancer), then it would be a good idea to speak with a migration agent.
