I stand corrected, but your wife will need to submit her birth certificate and marriage certificate as evidence of a name change. If your wife changed her name from birth by deed poll, for e.g. before you were married; then the birth, deed poll and marriage certificates are required. It's basically a 'chain of evidence' regardings all name changes, so if she were previously married then the divorce certificate would also need to be submitted. Men rarely change their surnames when getting married so you just need to prove you are married - women usually change their names when getting married so we need evidence of a name change, it just so happens that the marriage certificate is also proof of marriage.
