In relation to Q 26 - Is this where u declare all known medical conditions of just prescription medication. Do u need a letter form GP as I don't see any section where u can fill in GP details. Also anybody know what happens in the following situations 1. What happens if u don't declare known medical conditions? 2. calNgary Posted April 6, 2012 Share Posted April 6, 2012 I cant recall the form question by question as it was a few years ago when we filled it in. I do recall having to put down any exsisting conditions and as with anything visa related its not worth trying to hide something as they would probably find out,lol.. I enclosed a letter from specilaist even though it wasnt requested and i also brought over 6 mths worth of meds with me accompanied by a script in my name listing the type of medication when we actually flew.
