Guest heather Posted November 10, 2005 Share Posted November 10, 2005 On the form 80 you have to put in parents DOB. My partner was adopted and both parents been dead 15 & 20 years. He does not know their DOB. Does anyone know if this information is essential or if we can just put a note on the form to let them know this.
