ACS work reference - company letterhead

So the work references need to be signed on company letterhead... what exactly is considered a letterhead in Australia, what kind of requirement need to be met. i can get papers from former employer stating that i indeed used to work for them, but ive heard people saying theirs were declined because the lack of letterhead.

Questions:

1. does that still happen?

2. why is it so important?

3. what requirements need to be met for the letterhead to be legit?

4. what if the company simply does not use letterheads?
