mhaffie Posted January 25, 2018 Share Posted January 25, 2018 I have a birth certificate showing my birth name ... my mother re married and my father adopted me, my name changed and I had a new certificate do I need to show both certificates, or the latter one, the name change happened nearly 40 years ago. ali Posted January 25, 2018 Share Posted January 25, 2018 It's been awhile but when we applied we had to show birth certificate and any other legal documents that showed a name change (e.g. marriage certificate), so for you I would show both.
