nic1171 Posted July 13, 2013 Share Posted July 13, 2013 I had to print off forms 26 and 160, just google 'Australian medical forms and the numbers. Form 26 has a section to put in your GPs details but I was told this is just so they can get in touch if any abnormalities show during medical. Form 160 is for your chest xray.
