Ozmaniac
If the job is in a regional area, the main difference for employer/employee is that on an RSMS, you are required to stay with the sponsoring employer for 2 years or risk having your visa cancelled. On an ENS, you have a moral obligation, but are actually free to leave the employer at any time without risk to your visa. Cost to the employee is the same for ENS and RSMS. For an employer, there is no nomination charge for RSMS but the Regional Certifying Body may impose a charge - probably works out much the same. As far as the timescales are concerned, the actual visa grant time should be similar as both are currently in the processing priority category (Cat. 1) but the employee must get skills assessment for an ENS and that can take months for some occupations.
