When I was filling the form for health declaration where you are asked about your medical history. I mistakenly entered No to a question in which I had to mention that I'm pregnant. Cerberus1 Posted November 4, 2017 Share Posted November 4, 2017 The following form https://www.border.gov.au/Forms/Documents/1023.pdf is used to notify the Dept of Immigration that you need to correct an incorrect answer on your visa application.
