I went to a job expo last week and she was saying it had to be witnessed by a ward manager, but reading the AHPRA form it just mentions the letter head from HR etc to be signed and dated by them and CV just says to sign and date as true - eluding it has to be by the writer. Mummy Mann Posted March 14, 2013 Share Posted March 14, 2013 I got my ward manager to sign it and also to put her contact details. I also signed it as well to confirm it was correct.
