Doubts in filling Form 47PA for Contributory Parent Visa....Help needed.!

Hi, I am filling the Form 47PA for CPV. I have some doubts while filling this Form.

 

Question 3 Part B - Withdrawal of parent Visa application.

Since I have not applied to any Visa. Do I still need to answer this question?

These are the following indent questions in doubt. Blank space is left to answer by me and the answers are in bold letter. Please correct if I am wrong.

 

 

 

• On this application form, I am making an application for a class ______ parent visa. (143)
• I have previously made an application for the following parent visa classes_______ _______(not applied previously, shall I will leave this blank)
  
• Signature of the main applicant. _________ (Do I still need to sign here even if I am applying first time)
  

 

Question 11 Part D - While filling the Visa Number.

Visa number:My visa number is V589<0180772T1 but I can only write upto V589<0180772T Shall I write the last number outside the box or just leave it.
