adamexmouth Posted October 16, 2012 Share Posted October 16, 2012 Hello everyone, I have recently changed my name back to my birth name and am not sure which of the above forms I need to complete?. They both have sections on change of name? also on the first section where it says Family name, do I put down my new surname or old one?.
