PVM / Partner Visa Statatory Declaration

Hello,

 

For the PVM / Partner Visa, are there any forms for the statatory declarations of OTHER family members / people to fill in. Where would I find these forms or acquire them from?

 

and by that I mean the ones that family members fill in detailing that your relationship is real etc. (I'm not exactly sure if they are different from the ones that a teacher/doctor etc fills in like this one http://www.ag.gov.au/Statutorydeclarations/Documents/Statutory%20Declaration%20PDF.PDF ).
