I've been trying for months now; the first form was lost, second form was rejected as the qrops form changed in April (form arrived in March but they did not process until after April). Find the telephone number for the HMRC (QROPS) department that signs off on them, call them direct, that's what we did. We got through to a guy that took control and processed it for us.
