I had my last name changed around 17 years ago when my mother married and I was legally adopted by her husband and took his last name. It states in the document checklist to upload 'evidence of name change'. Has anyone had experience with this or knows what evidence I would need to provide? Guest Posted September 22, 2014 Share Posted September 22, 2014 I would think in this case an adoption certificate would be the easiest, (or other legal papers of the adoption) otherwise you can get a statutory declaration. My OH has no birth certificate and his adoption certificate was perfectly accepetable. I changed my name 'by usage' and had no documentary evidence - it had never cause me any problems in the UK but changing your name 'by usage' doesn't seem to be understood in Australia and it did cause me some hiccups - not least that I was told I would need to take my driving test again because my driving licence and birth certificate were in different names. In my case it was a tiny change and the name I changed to is an commonly accepted derivative of my birth certificate name! Link to comment Share on other sites More sharing options...
