Hm, why do they have space for 19 digits on the form then? Visa/MC are only 16 digits, Amex is 15 ...

I guess most people pay by credit card. On paper application they have a form field for credit card number only, but not for an additional 3 digits from the back of the card. I think these 3 digits are required for all 'card not present' transactions? Do I need to add these 3 digits to a credit card number?

In these forms both applicant and authorised recipient must put the date and sign the form. Which date should they use, date when they sign it? If authorised recipient is in Australia and applicant is overseas, dates would be different of course.
