I am using form 888 for the main two but wondered if I had to use an official form for the others or if I could just to print out sign and certify a word doc. Guest Posted April 28, 2014 Share Posted April 28, 2014 Afaik they have to be originals. All have to be on the 888 if they are stat decs from Australians. They need to get them witnessed as per the rules and regs in Aus (if they are being sent from there). So copy of passport and a certified person signing off on it etc. If you have 2 that is all you are required to submit isn't it?
