My wife's last name has changed after marriage.. so basically her birth certificate has her birth surname and her passport has my surname... What doc can we give to fulfill this criteria? Thank you! No (or at least my wife didn't need it on her marriage certificate). You do need to take the original to the citizenship interview/test.
