We have just had our 491 visa grant through after being invited by SA at the end of June. Rather quick turnaround on the application, which could be down to the occupation we applied under. If your application is rejected, there is no bar on the number of times you can reapply. So the best plan is just to get going and apply. In general, though, you need to be on a permanent visa OR in a priority occupation (i.e. medical) to get an exemption. If you are on a permanent resident visa (so not a 491 which is provisional) you don't need a travel exemption to travel to australia as far as I was aware...
