Anyone please list the Identity proof which is eligible for Application. I have the below identity proof with me 1) Medicare card 2) Age proof card (With Photo) 3) Key pass card (With photo) 4) Bank Statement 5) Over seas DOB certificate(Translated in Australian format) 6) Over seas Passport. Sometimes they ask to see power bills etc, rate notices but if you are not a home owner that would be hard. I googled it and this came up You will need to provide: full birth certificate showing parent's details. passport or travel document. evidence of change of name if needed. a passport-sized photograph. identity declaration signed by an Australian citizen with the correct authority. Dec 18, 2019 Hi Friend, Thanks for your kind reply. Recently I translated my Birth Certificate using Australian Migration translation service. But My mom Name has a spelling mistake.
