Guest Watersports
Small "yipee" Got asked for our medicals today. However in the email from our case officer it says to download form 26 and form 160 but when I go to them they both say for onshore use only. When I read up on the website about forms for off-shore applicants it says my case officer will send them to me. HELP!
Guest spray21
I remember struggling to find the correct forms - the ones for off-shore applicants. I would attach them, but I joyfully deleted all my forms from the computer the other day, hoping to never need them again! They do exist online, if you can find the right combination of words to search for. Failing that, DIMIA can post them to you - depending on how fast you intend to have the medicals done - or some medical centres have copies of the forms themselves, but check this v carefully. (We wasted a journey to a place that did have one spare form but it was missing its final page, so no use!) Don't give up on it, you'll get there! Guest spray21
FOUND IT! http://www.uk.embassy.gov.au/lhlh/files/26%2epdf Knew it was there somewhere! There are lots of sites saying that these forms are only available from DIMIA, or only from your CO - not true! And here is the xray form: http://www.uk.embassy.gov.au/lhlh/files/160%2epdf And from this link http://www.uk.embassy.gov.au/lhlh/health.html you can get to lists of panel doctors etc. (Who have to carry out the medical exam). Make sure you shop around for prices, as they can vary a lot. Have fun!
Guest Watersports
Great - thank you so much! Can't believe a case office sends in-correct info. I was quick off the mark and booked an appointment first thing this morning and then afterwards looked for the forms. With the appointment only a couple of days away I was starting to panic....especially as it's £50 per person cancellation fee. Phew! Guest spray21
No worries! I knew all my hours online sorting all this cr*p out would pay off in the end! (Am seriously considering setting up some sort of migration consultancy service once I finally get over there!)
ali
The medical forms were attatched to our email the case officer sent - we didn't need to go to another site to down load them Ali
Guest Watersports
Lucky you - I think you got a better case officer then me. I just double checked and mine says: All forms referred to in our letters (excluding those for medicals) can bedownloaded from http://www.immi.gov.au/allforms/index.htm OR from thewebsite indicated in attached correspondence. And the websites indicated all relate to onshore applications only!
restfamily
gosh that is dreadful, shounds like we had a great case officer. We never had to email them and only had three emails from them: one to say hi; one for the medicals and one with the decision. Sounds like pot luck!
