I am unable to get a statement of service from two of my employers so I'm doing the statement of declaration with supporting evidence. My question is on the statement of declaration it says to state why are you unable to get a statement of service, and I'm not sure what to say. My current job I don't want to jeopardize, and another employer my manager no longer works there. saslbr Posted January 8, 2013 Share Posted January 8, 2013 For statuatory declaration there is a fixed format that you need to use, please google New South Wales (Latest) statuatory declaration form. Don't forget to use the word latest as. This form will help you. You need to have it attested by an officer such as Police officer, Bank Officer, Justice of peace, etc etc its all mentioned on the form.
