Guest Gollywobbler Posted December 17, 2007 Posted December 17, 2007 Hi there What does the e-mail say and what is the rubric on the form describe this Form 1091 as? Form 1091 is not available via the DIAC website. After all, the e-mail doesn't say, "Here's Form 1091. Take it to the Panel Doctor" without specifying or at least indicaitng - why they want you to do this. We need more info please in order to help. Are the words "Health Undertaking" mentioned either in the e-mail or on the form? Best wishes Gill

Best wishes Gill the email is via our agent but reads that further medical information is required for oh and must be done by same panel doctor and to take form 1091 which is called Medical reports /tests for an australian visa
Guest Gollywobbler Posted December 18, 2007 Posted December 18, 2007 Hi there Hmmmm. Has OH got a known medical condition - eg high blood pressure or imilar? What do they want him to have done by way of furher tests or an examination by a specialist? I wouldn't worry, though. The MOC requests additional tests and/or specialists' reports at the drop of a hat. It almost never affects the grant of the visa in th end, though. Cheers Gill
calNgary Posted December 18, 2007 Posted December 18, 2007 I was sent for extra test but had to go and see a panel specialst rather than panel Dr,,I worried my 'bum' off but shouldnt have as we got the visa in the end,,it just delayed things by a few weeks,,good luck and fingers crossed for you Cal x
Guest ASPIS4 Posted December 18, 2007 Posted December 18, 2007 OH has R Arthritis and the kind of things they want the panel doctor to answer is what are the likely costs to the OZ health going to be, what is panel doctors opinion of prognosis etc etc, really annoying as could have done all this at initial medical as aware of condition and we supplied letter from consultant, doctors records etc etc
