    Form 815 Health Undertaking

    Hi everyone. Verity and I are now in the final stages ofour application with police checks and medicals completed an sent off. From our CO we have now received form 815 Health Undertaking She has been asked to sign it and send it back but we just wanted to know what it was all about. Reading through it, it states that upon arrival in oz we must contact the Health Undertaking Service within 7 days. Looking through the form it does seem to focus a lot on tuberculosis which verity does not have. It does mention "or other health conditions of concern"- welll verity has a prolactinoma (a small growth on her Pituitary gland)- so assume it could be for this? Although to us it is not a concern as all it essentially means is she has a hormone imbalance whereby her would body produce breast milk - even when not pregnant- but this is controlled by a weekly tablet. Has anyone else had this before? Thanks Ben
  2. hi everyone, have a query which i hope some one will be able to help me with. we are in Oz and applying for ENS visa. Went for my medical and have received a letter referring me to the health undertaking service for TB testing. Now, this has come as a shock and am left wondering where this leaves us with our application. I do have an abnormal x-ray due to a bad case of pneumonia. This was declared for our last visa and was passed through the radiologist and then LCU with no probs. Thought there may have been an issue then, but since there wasn't put it out of my mind. Now, after medical here, again declaring the info and showing letter from my old consultant regarding the pneumonia I have received this referral and need to attend the chest clinic as a suspected TB case. Have never had TB, have been immunised against it, and only have abnormal xray due to lung scarring after the pneumonia, which I have already demonstrated to them. Am obviously really concerned now that this will affect our visa application and am wondering if anyone has experienced this? many thanks

    Form 815 Health Undertaking

    I received form 815 from my CO. I don't know the exact reason. Does anybody has the same experience?
  4. I have received a request from my Case Officer to complete a Health Undertaking Form (Form 815). As I am applying outside of Australia I’ve been told I MUST provide “Contact details in Australia”. Unfortunately I do not know anyone in Australia, I don’t know anyone in the UK who knows anyone in Australia. Would someone living in Australia be prepared to be my initial contact should immigration decide to send me a letter? Obviously once I’m in Australia I will contact them and given them my own address. I know other people have asked similar questions and have been told to leave this section blank, but my Case Office insists that it MUST be completed. Please helllllllllllp.
  5. Health undertaking to sign -How many days for visa? Hi all, i have been asked by my CO to sign health undertaking as my chest x-ray was abnormal. All other documents show status " Met". Any idea , how many days/weeks it takes for visa grant after i submit the signed undetaking. thanks

  7. Hi All, First and foremost I am not a British but an Indian living in Dubai. My visa has been granted and I have to enter OZ before July 18th 2010 with an undertaking to contact the Health Undertaking Department within 4 weeks of arrival. I have a distance past history of TB (17+ years) and have gone through the complete medication course however, the scars remain there. I am currently working in Dubai and just want to land in Australia to validate my visa. I have following questions: Will I need to stay in Australia until all the tests are done and reports received? Can I take an appointment on email and plan my travel and accordingly. If I cannot take an appointment via email then what is the process and time frame of getting appointments in cities like Melbourne or Sydney. Is it mandatory to report to Health Undertaking Department on first short visit to Australia or it can be done later on second/third longer visits. Thanks in advance. VB
  8. Dear, I have a short question. I have seen that onshore applicant needs to submit form 815 (Health Undertaking) after they have pre-grant visa but with the offshore applicant when they need to sign it? Is it after a decission is made or before even a CO is assigned? I just want to know the process. If anyone knows please let us know. Thanks and regards
  9. Hey, everyone, I have received last month my OZ visa grant (for Spouse Provisional, Subclass 309) and was advised to enter OZ before mid May to validate the visa. But my Aussie husband just recently left OZ for Hong Kong because of job change and we won’t be back to OZ permanently in about 2 years. So I will stay in OZ for only few days after entering in April or early May to validate the visa. In this case, do I have to contact the health authority for the health undertaking, OR, just do it in the future when I settle permanently? (The visa grant letter does offer a free phone number for the health undertaking service, but it seems only for health-check appointment booking, not for inquiry like mine). Does anyone have experience to share? Much appreciated! Tina
  10. Hi all, was expecting to get our visas this week but guess what No! Had an Email, got all excited but alas it was to fill in a Health Undertaking form as our daughter has had contact with TB. Although passed medical, negative chest xray, and heath test she now has to be put under surveillance once we enter Australia. We have to contact Health Authority everytime we leave country or move house. I am not saying this is a bad thing, I think this country can learn a lot by screening for TB, but just want to get visa!!!! Also says we have to validate by June 18th police checks and medicals were only done in November this seems really quick.:mad: Feeling really stressed now! Jo
  11. Got an email this morning..... Not the one we were hoping for but not bad news at least. I have to agree to have a medical in Austraila within 4 weeks of arriving to double, triple check i don't have TB! But our agent says once they have the form back then the VISA's will be on the way. We have sold the house and will be moving into rented digs for the last few months. A one bedroom flat - which will be interesting with our one year old! The move date for the house is a week on Wednesday and we are hoping to be in Brissy for September. What a journey... I need a rest from forms, worry, medicals, more worry, guilt for leaving the family behind, and the dreaded waiting................
