which medical form??

I know this may be on the site but I can not find it so I would really appreciate being pointed in the right direction.We are going for our medicals soon. Please can you confirm which form we have to fill in to take with us. Thanks

hi kc good to hear from you . you need to take form 160 for xray to be done 1st and then form 26 for meds. hope this helps

I am still in the UK and when I looked up form 26 on the DIMA website it said for those applying in Australia. I am missing someting?It is late on a Tuesday evening!!!!

Look on the uk Australia High Commision web site and they are on there.

Go to visa section and at the bottom of the page it says medical forms, both 26 and 160 are there.

hi kc I am still in the uk and have just had my xray & meds done last week and the above forms are what we took with us
