Jo, I didn't look at the immi site on the friday we got our visa, had a quick look saturday and lo and behold there it was lolx

Mongoose send me your email address and I'll send the form to you, think they've taken it off the list on the immi/diac site but I've got one on my computer if you want one.

Take care and thanks again for your replies, just about alive this morning, and yes we did do alot of celebrating...hic..hic...lolx
