I'd never heard of a 1399 form and it isn't on the normal DIBP list of forms. I hunted for ages for info on it and the best I could come up with is that it's a 'Declaration of Service' form but wasn't able to determine what it was for or get a copy of the form itself. It isn't often asked for but that's all I could find out. At least none of the info I found indicated that anyone who was sent the form had a problem with their application so I'm sure it's nothing to worry about.

You'll obviously have to complete and return it but for future reference, can you tell us what it was about? And let us know the outcome of your application?

It sounds like you must have answered Yes on your permanent visa application to a question relating having served in a military force. If that's the case, it would explain why not many people need to complete a Form 1399.

They asked me to fill this form when I applied for my citizenship back in June 2014.

My answer to the question was : I have never served or involved in any military activities. Conscription is not exist in my country and I was exempted from conscription during my time in Singapore. As proof of evidence, i gave them a letter of exemption from the Ministry of Defence.

You don't have to provide an evidence if conscription is not exist in your country, but if you have served in the army before that will be another story then.

I submitted my exemption certificate as they know that conscription is exist in Singapore.
