Where are the police check forms?

Hi everyone, I remember reading a thread about how to get these forms on line some time ago, I have searched but can't find it. Can anyone post a link? If it matters by region - we're Lancashire.

The only link I have is for Thames Valley Police. I had a quick look on the Lancashire Police website but it drew a blank when I searched for forms. Hiya i rang Manchester head office and the lady e-mailed them to me there and then ,we filled them in and took them to local station with fee ,,,thats when the wait began a full 40 days ,,seemed an eternity
