Stat Dec.

Hey all, been trying to get work ref etc.. to prove my work experience with a boss from Australia, but am having no joy in getting anything from him. So it is time to do a stat dec, I have found stat dec templates for Australia, but I am in Ireland so was wondering what sort of format/template for a Stat Dec should I do? And can I get the stat dec witnessed and signed&stamped by a Police Officer?
