If you decide for choosing an Agent I saw an offer for $1500 the Australian Migration Agent and Immigration Lawyer Association website (ask-an-agent). Maybe you can use this as a benchmark.

Yes, you must be offshore when the visa is granted. I think there is no chance to avoid the medical if you want to enter on your whv. Good luck!
