Statutory declaration

Hi guys, can anyone tell me how to get a statutory declaration form off the internet to allow me to deal with my old employers that dont exist.On the TRA form it says "you may submit a personal statement on a properly signed and witnessed Statutory Declaration."
Where do you get these forms from.

Statutory Declaration

I (name) of (address) do solemnly and sincerely declare as follows:

why you are do this stat dec........................

who you have worked for.....................

what role and responsibility

list your day to day tasks and duties

list all the tools and equipment you use from a pen to the highest

any training you received from this company

I make this solemn declaration believing the same to be true and by virtue of the Statutory Declaration Act 1835.

Signed: by you

Declared at: solicitors address

On this day: date

Before me: solicitors name

Signed: solicitors signature

Cheers for that kp nuts, do you need to use one of these for all missing employers or one for each.

I think you can just do the one, but break it down to the years you need if for.
Then put the years in that you do have references with company name and dates, saying see attached reference, so you have a full working history on it.

Hope that makes sense.

We did our's because oh is self employed, and also his first main company is no longer trading, and we put the whole lot on one stat dec, it was 4 1/2 pages long....

Hi kp nuts, thanks for the statutory decloration advise, one question though, on the TRA section 8 i have to fill in and explain all my past history with a complete detailed write up. This i have done but do you need to put it an the statutory decloration as well?

Hi there again, you can do i think, you can do the whole stat dec as a working history from start to finish, and if you have any references from any compaines at the bottom of that section, say see attached reference ........

Just remember the more information the better

What pathway are you using???? If it's pathway B you will need to show all your training and qualifications and attached any certificates you may have, (but I'm sure you know that one).

Hi kp nuts, These pathways confuse me a little, i am an upholsterer, and i have been told to use pathway D, what so you recomend.

Noooooo, pathway d is cancelled at the moment. They cancelled it last September time because they were having a lot of fraudent applications.
