I need some assistance

Hy all,
I spoke to Visa Bureau this afternoon regarding my application, i will need to put together what they termed a " Work History Statement "
Can anyone offer some pointers as to the layout i need to submit, or is willing to email me one they have submitted so i can use it as a template.

Hi, we used the Visa Bureau and they were very helpful with all our application. My husband sais he did a Work History Statement but he didn't keep a copy unfortunately. He said he just wrote an essay on every little thing he did from the first job he had till his current job. I think they gave him a template to follow but can't be sure.
