Hi, there is a form, and yes he needs to sign it. Fortunately I get on with my ex and he signed no problem. He needs to sign it and get it witnessed. I have sole custody of my two girls and he has regular access, but we still had to get his permission, bizzar when I have sole custody but its part of the process unfortunately, so good luck with that.
I hope it goes smoothly for you, I told him we were going well in advance of starting the visa process so it wasnt a shock when I produced the papers.
